Health Care Transition

Health care transition is the deliberate process of moving seamlessly from child-oriented health care to adult-oriented health care. An integral aspect includes the ability to be the leader in one's own health management.

Health care transition is important for all children and young adults, but it is critical for those with CMCD. More than 600,000 youth with CMCD enter adulthood every year, and along with their newfound status, they find they have to navigate a complex system of care that is dysfunctional. All the supports they had as children suddenly disappear and health care becomes an all-encompassing obstacle. Obtaining and maintaining insurance is difficult. Maintaining optimal health so one can pursue college or work is a challenge. Finding doctors who are able and willing to care for childhood-onset conditions is next to impossible. Health care transition planning is non-existent, in spite of national recommendations since 1989. Without health, young adults with CMCD, cannot realize their full potential and contribute meaningfully to society.

PPC developed a white paper on behalf of the Coalition for Young Adults Living with Chronic Medical Conditions and Disabilities to describe the state of health and health care for these young people.

PPC convened the first meeting for the Coalition of Young Adults Living with Chronic Medical Conditions & Disabilities in June 2009. Some of the coalition members met to provide input for the white paper.
