For those of you who have not read our (lengthy) white paper, PPC is working on improving health care transition for young adults with chronic medical conditions & disabilities (CMCD). Young adults, 18-30 years, need to be in optimal health so they can live learn, work, live and love. Parents and others who work with young people with CMCD are constantly amazed by their grace, will, and strength to overcome barriers and achieve their dreams. Society has yet to acknowledge the leadership that young people with CMCD can provide to both elders and youngsters.
