Washington, D.C. (Nov. 10, 2009) – A recently released white paper calls on Congress to address the often overlooked area of health care transition for youth and young adults living with chronic medical conditions and disabilities (CMCD). Appropriate health care transition ensures a seamless transition from child-oriented health care to adult-oriented health care.“Currently there are 15 million youths and young adults living with CMCD, and as the prevalence of chronic disease grows this number will only increase.” said Dr. Santi Bhagat, founder and president of Physician-Parent Caregivers, Inc. (PPC). “Yet most adult physicians have not been trained to care for adults who have childhood-onset CMCD, leaving young people with unmet medical needs and struggling to find appropriate health care providers.”
